Forms for Admission to PTA

In order to complete the PTA Program Application you will need electronic copies of the following documents:

• Transcripts from all schools attended
• Substitution Course Form (if applicable)
• Courses in Progress Form (if applicable)
• HESI results sheet
• Essential Job Functions Form
• Observation Hours Forms
• Essay

The required documentation that must be physically mailed or hand delivered will be the following:

• Reference Forms (2)
• Student Observer Evaluation Forms
