Your parents’ 2020 tax return transcript or 2020 income tax return (i.e. 1040 with all schedules, if applicable) is required when submitting your scholarship application. If submitting the return itself, one of the tax payers' signatures must be present. The completed application, with all supporting documentation and signatures, can be emailed to Dr. Miles Young at email@example.com; or, students may submit their completed application to their counselor.
